On the day of each training session, it is important that you complete the Covid Return to Play form on behalf of your child.

This is a very simple process, and when you complete the form for the first time, the app will remember your answers. For all subsequent events, all you will need to do is scroll down and press the SUBMIT FORM button at the bottom of the screen
